Only the first 3 fields are required, but feel free to let me know more details, including your thoughts on the composition if you so choose.  I will eventually be adding a performance location map to this site (similar to the one I have posted for my collaborations) so your city will be added there.  Any comments you might provide could be considered for the quotes section of this site, so for proper attribution, I would need your name and title as either Conductor or Director.  And, of course, I always want feedback on my work.  Lastly, if you want me to notify you when I add a similar composition to my library, your email would be most helpful.
